What is LifeBox, why is my hospital using LifeBox and how do I use it

What is LifeBox?

This Online Service was created by Doctors and Patients in 2018 to make
the pre and post-operative process simpler, safer and easier, and to allow
you to complete as much of the process as possible in the comfort of your own
home.

It’s been designed so that you can securely share information with your
nursing team, and to act as a safe and easy-to-access place to store the
medical information that’s relevant to your procedure.

What do I use LifeBox for?

Before your procedure, you’ll need to complete a health questionnaire,
covering your medical history, lifestyle, circumstances, and current
medication. You’ll also find some informative videos and there may also be some
important documents to read, that have come directly from your hospital. It’s easy to fill in; you can stop and start when you need to and don’t have to do it alone.

What happens once complete my tasks?

Once your questionnaire is reviewed, your hospital will get in touch to
explain the next steps. And they might also ask you to clarify a few
things. This is just so they can be absolutely sure they have everything they need
in order to make your care, as safe, quick and comfortable as possible.

When should I use LifeBox?

To help the hospital help you, it’s really important that you do it as soon as
you can and that the information you give is accurate and up to date, as
it’s going to be used to assess your suitability for surgery and to identify
your medical needs before, during and after your operation.
