Why do you need to activate your account, what you need to activate and a step by step guide to activate your account.

Why do you need to activate your account?

To prepare for your pre assessment appointment, you are asked to activate your LifeBox account. Once you have activated your account you will be able to complete your Health Questionnaire, watch information videos relating to your procedure and view any files that have been uploaded by your hospital. You only need to activate your account once.

What do you need to activate your account?

Your hospital will create a LifeBox profile for you using information they hold on file, including your email address and mobile number.

LifeBox follows a two-factor authentication process to ensure the information held on your account is secure. This process includes an email containing a personal activation link, and a text message containing a unique confirmation code.

Step One - Using your personal activation link

Once the hospital has created your profile, you will receive an email from the hospital & LifeBox.

In the email, you will see a square blue button which says "Activate your account" This is your personal activation link.

When you click the blue button, you will be taken to a browser where you can continue the activation process.

Step Two - Creating your password

When the browser opens, you will be taken to a page with the heading 'Activate LifeBox account.' Your email address will already be shown in the 'Email' field.

You will be required to create a password for your account. Please type this password in the "PASSWORD" field and confirm it again in the "CONFIRM PASSWORD" field.

Then click "SIGN UP"

NOTE: Your password must include a minimum of 8 characters, a capital letter, a lowercase letter, a number and a symbol. If you click on 'password requirements' you will see a list of requirements to meet the password criteria.

An example of a password is 'Lifebox2?' - There is a capital letter 'L', a number '2' and a '?' symbol. Your password is complete when you see a green tick next to each requirement.

Step Three - Receiving your confirmation code

Clicking 'Sign up' will direct you to a page with the heading 'Confirm activation'

The LifeBox system will automatically send a confirmation code to your mobile via SMS.

Enter the six-digit confirmation code that is sent to your mobile and click "CONFIRM"

NOTE:

  • The code will expire after 24 hours

  • Ensure your phone is switched in and has a signal to your network carrier

  • Allow sufficient time for the system to send the code

  • If you do not receive your confirmation code, try clicking 'Resend' found under the 'Confirm' button

  • The code is unique and we are unable to provide the code manually

  • If you have checked the above and still have not received a code, contact the hospital to check that the right number is linked to your LifeBox profile

Step Four - Signing in to LifeBox

Clicking 'Confirm' will direct you back to the main LifeBox login page.

Enter your email address and the password you set up during Step Two into the “PASSWORD” field and click “Sign in”

Step Five - Accepting our Privacy Policy

Read our Privacy Policy carefully. If you agree to the policy, tick to confirm and click 'Next'

Your LifeBox account is now activated. You will be able to complete your Health Questionnaire, watch information videos relating to your procedure and view any files that have been uploaded by your hospital.
