When would I reset my password?

If you have previously activated your LifeBox account, you may need to reset your password if you can no longer access it.

How do I reset my password?

You will need to visit the LifeBox homepage and have your mobile phone handy to receive a confirmation code in order to complete the reset password process.

Enter the email address associated with your LifeBox account in the field marked 'Email' and click continue.

Click forgot password underneath the sign-in button.

Re-enter your email address. An email will be sent to your email address containing a link to reset your password. In the email, click reset password and you will be directed to the LifeBox app.

If you do not receive an email, make sure you have typed the correct email address and check your junk/spam folder.

A confirmation code will be sent as an SMS text message to the mobile number associated with your LifeBox account. Enter the code, and type a new password.

NOTE: Your password must include a minimum of 8 characters, a capital letter, a lowercase letter, a number and a symbol. If you click on 'password requirements' you will see a list of requirements to meet the password criteria.

An example of a password is 'Lifebox2?' - There is a capital letter 'L', a number '2' and a '?' symbol. Your password is complete when you see a green tick next to each requirement.

When all fields are complete, click the “Reset Password” button

You will be taken to the LifeBox login page, use your new password to log in. You can now access your health questionnaire, view information videos relating to your procedure and access files that the hospital may have uploaded for you to view.
